Der is mercury in d thermometer. Solids expand wen dey r heated & contract wen dey r culed. Wen v kip d thermometer under d tong d mercury expands & v can no our body temperature.

So that the air from the surroundings  does not pass over it and cool it thereby giving an incorrect reading ,we need to keep it in our mouth . Keeping under our hand may give a wrong result if we didn't kept correctly .
