This is a very warm part of the body, close to the actual internal temperature of the body, and therefore a more accurate place to check; besides, the norm of 98.6 is based on the oral temperature, so this is a better place to provide a basis of comparison. It is placed under the tongue rather than above to minimize the cooling effect of air being breathed in and out during the reading.

- The intense heat the body has will be in thermal equilibrium with some body parts like tongue, ear, armpit etc.For convenience and accuracy always tongue is selected for measurement.
- There is good supply of blood in tissues under the tongue.

Traditionally rectal temperatures were the most reliable, but patients did not like doing it that way. Under the armpits is not reliable. The mouth is a compromise. Under the tongue keeps us from cooling the thermometer with our breath. Tympanic membrane (eardrum) temperature measurement got popular, but results are not as reliable as oral or rectal. Skin (temporal artery) readings may be above or below rectal temperatures.
Back to tradition. Rectal is best. Okay to substitute oral with the sensor under the tongue and no recent beverage - hot or cold.
