Four points to be kept in mind while preparing the questionnaire/ interview schedule :
1. Questionnaire should have limited number of questions. Do not ask too much questions either its a questionnaire or interview
2. The questions asked should be simple and easy to understand. for example - Use "Marital status" instead of "Are you married or not ?"
3. The questions should be arranged in logical order. For example - Use this sequence "Name than age than marital status" , do not use this "Marital status than age than name".
4. Avoid asking personal question like At what age will you marry? either in questionnaire or in interview.
